Did you try to let the contract expire? may be the game will assign you a "starting" doctor with a salary of 50m.

Damn, it is harder to buy a Doctor with rating under lvl 5 than it is to get one lvl 18. Someone must have a lvl 1 doctor they can sell me.

Just get rid of your doctor.  The game gives you a 1/2 star one.  It's not hard
